Two Conflicting DGHS Orders in 48 Hours
New Delhi: The Central Government has withdrawn its directive that barred physiotherapists from using the ‘Dr’ prefix, creating confusion after two contradictory communications were issued by the Directorate General of Health Services (DGHS) within 48 hours.
On September 9, DGHS wrote to the Indian Medical Association (IMA) declaring that physiotherapists cannot use “Dr,” citing a violation of the Indian Medical Degrees Act, 1916. However, on September 10, DGHS withdrew this communication, stating the issue requires “further examination and deliberation.”
Background: NCAHP Recommendations
The debate began earlier this year when the National Commission for Allied and Healthcare Professions (NCAHP) recommended that physiotherapists be allowed to use the “Dr” prefix with a “PT” suffix in its Competency-Based Curriculum for Physiotherapy 2025.
This sparked objections from several medical associations, particularly the Indian Association of Physical Medicine and Rehabilitation (IAPMR), which argued that physiotherapists are not medical doctors and their use of “Dr” could mislead patients.
Objections from Medical Community
In its September 9 letter, DGHS cited legal precedents and past rulings, including those from the Patna High Court (2016) and a Bengaluru court (2020), which prohibited physiotherapists from using “Dr.” prefix. It also referenced advisories from the Tamil Nadu Medical Council and earlier decisions of the Ethics Committee of the Paramedical & Physiotherapy Council Bill (2007).
The letter warned that using the title “Doctor” without a recognized medical qualification amounts to legal violation under the Indian Medical Degrees Act, 1916, punishable under Section 7.
Withdrawal After Backlash
Following strong representations, DGHS issued another letter on September 10, withdrawing its earlier stance. The clarification stated that the matter requires “further examination” before a final decision is made.
This reversal has left the issue unresolved, with physiotherapists, medical practitioners, and allied health professionals awaiting a final directive.
Recognition of Physiotherapists’ Role
Amid the controversy, on World Physiotherapy Day (September 8), Prime Minister Narendra Modi praised physiotherapists, acknowledging their contribution in improving mobility, dignity, and quality of life, especially for the elderly.
